ACBIS Application (Groups of 5-29 Applicants)
Certified Brain Injury Specialist/Certified Brain Injury Specialist Trainer Application Fee for groups of 5-29, including complimentary one-year subscription to the Journal of Head Trauma Rehabilitation.

Each application also includes a one year subscription to the Journal of Head Trauma Rehabilitation (JHTR). Each applicant must provide BIAA with a home address in order to ensure that the JHTR is delivered to him/her. If a work address is provided, the JHTR will be mailed to that address and may not get to the applicant, depending upon the organization's mail processing procedures.
Any subscriptions sent to a business address that are not received are not subject to be replaced. If you are not receiving the JHTR subscription, contact BIAA within 60 days of the missed issue. If you move during your annual subscription, contact BIAA with your updated address within 90 days. BIAA is not responsible for providing any missing issues of the JHTR during your one-year subscription if you fail to meet any of the above requests. For more information, click here.
Application fees are not refundable. Individuals seeking fee adjustments must request them within two weeks of payment.
